Medical necessity for residential mental health treatment under Cigna is typically established by clinical assessment showing that lower levels of care (outpatient, intensive outpatient, partial hospitalization) are not clinically appropriate or have been unsuccessful. Evernorth Behavioral Health applies medical necessity criteria consistent with the American Society of Addiction Medicine (ASAM) criteria and the LOCUS or CASII level-of-care criteria for mental health.
